The following requirements are needed from all subcontractors:

  • General Liability Certificate 

  • Workers' Compensation Certificate 

  • Auto Liability Insurance for all “owned” “hired” Autos (or proof of personal insurance) 

  • Completed W-9 Form 

  • Subcontractor Agreement initialed and signed 

  • Vendor Information Sheet 

  • Certificate of Good Standing from the Colorado Secretary of State Website- If Registered 

  • Licenses: All Licenses must be current
